In-Person Vital Record Requests
You may obtain a certified copy of birth, death or marriage certificates in person at the North Kingstown Municipal Offices.

Mailed Record Requests
You may also obtain certified copies described above by mail, your request must be accompanied by:
- Business sized, self-addressed stamped envelope
- Check made payable to the Town of North Kingstown
- Copy of your driver's license
- Please also provide a daytime phone number or an e-mail address
Effective July 1, 2018 the following fees apply:
- In-Person requests: $22.00 for the first certified copy/$18.00 for each additional copy
- Mail requests: $25.00 for first certified copy/$18.00 for each additional copy

The North Kingstown Town Clerk’s Office maintains copies of birth records for births which occurred in North Kingstown, or for residents who were living in North Kingstown at the time of their birth. We can also provide copies of most birth certificates for anyone born in the state on or after January 1, 1960.
An applicant may request a certified copy of his or her own birth certificate or that of an immediate family member.

The North Kingstown Town Clerk’s Office only issues certified copies of death certificates of individuals who died within North Kingstown or who were residents of North Kingstown at the time of their death. Certified copies of death certificates will only be issued to next of kin or to persons with a legal right to the certificate.

The North Kingstown Town Clerk’s Office maintains copies of marriage licenses of marriages that occurred in North Kingstown or marriages of residents of North Kingstown. Certified copies of marriage licenses will only be issued to the parties to the marriage or persons with a legal right to the record.
